Patient Retention Risk Scoring Rubric
Each active patient without a future appointment receives a retention risk score (0–100+). Higher scores indicate greater urgency for outreach. The score combines time since last visit and no-show history — two key predictors of patient churn in orthodontic practices.
1Days Since Last Visit
How long since the patient was last seen (dismissed from an appointment).
Logic: Patients without any visit history receive 80 points. The longer the absence, the higher the risk.
120+ days — 95 pts90-120 days — 80 pts60-90 days — 60 pts<60 days — 30 pts
2No-Show History (12 months)
Number of no-show appointments in the past 12 months.
Logic: Each no-show adds 10 points. Patients who miss appointments repeatedly are at compounding risk of dropping out entirely.
Whether the patient has any scheduled future appointment.
Logic: If a future appointment exists, the patient is excluded from the alert board entirely (score = 0). A scheduled appointment means the patient is still engaged.
